			<content:encoded><![CDATA[<p>Trained language interpreters or bilingual healthcare providers serve critical roles in the delivery of care to patients with limited-English proficiency.  These staff provide invaluable service facilitating communication between healthcare providers and patients—often in the context of cultural differences and varying levels of health literacy—in addition to their primary job roles.  Although using trained staff is the best option for providing quality care and reducing errors, ad hoc <a href="http://lyriclabs.com.my">interpreting</a> by family members and untrained bilingual staff remains common practice within many institutions.</p>
<p>In response to these tremendous challenges to the delivery of culturally and linguistically competent care, Lyric labs retains bilingual linguists  to serve in dual roles as interpreters (e.g., registrars interpreting for patients and providers) through its Qualified Bilingual Interpreter (QBI) Program..  Each interpreter focuses on interpreting roles—conduit, clarifier, cultural broker, and advocate—and techniques as well as interactive role play and problem solving activities.   Within the Healthcare System, interpreting requirements are growing at a much faster phase. Lyric is constantly looking for more qualifed bilingual interpreters to serve the clients.</p>
<p>In 2010, Lyric initiated a four-phase evaluation plan to assess the impact of the  dual-role interpreters on language access and quality of care for limited English proficient (LEP) patients.  This formative evaluation was designed in four phases—interpreter engagement, provider engagement, patient engagement, and quality research—with each engagement phase contributing to the research infrastructure for the study of interpreters’ impact on quality of care.</p>
<p>For this presentation, we will describe focus group and survey findings on the experience of QBI interpreters, including perceived assets of the program, challenges to implementation, the impact of training on patient care, and the emergent roles of QBI interpreters.  Findings revealed that QBI interpreters, who primarily address linguistic barriers in a conduit role, increasingly address systems barriers, such as racism and discrimination, in the advocate role despite training recommendations that this role should be rarely used and patient-initiated.   More specifically, interpreters find themselves compelled to use their moral compasses to:  intervene directly or indirectly on behalf of patients when providers display culturally insensitive behavior; assume the role of health educator when prompted by a provider; fill in pertinent health information omitted by the provider; and inform patients of <em>non-medical </em>features unique to Malaysian healthcare (e.g., the billing process).</p>
<p>In summary, roles interpreters formerly perceived as supplemental to an interpreter’s conduit role are emerging as equally important to converting messages alone.   In our discussion, we will explore how QBI interpreters may be used as agents for organizational change as it pertains to cultural and linguistic competency.  As frontline observers of clinical practice, the experience and expertise of QBI interpreters could aid in the reformation of healthcare systems to meet the needs of a changing cultural and linguistic demographic in accordance with malaysian standards.</p>
